What Is Open Rhinoplasty?
Open rhinoplasty involves a small incision on the strip of skin between the nostrils, called the columella. This technique allows me to gently lift the skin and fully view the underlying nasal structures. Compared to the limited view provided by the closed approach, the open technique gives me direct access to the lower lateral and upper lateral cartilage, as well as the bony nasal pyramid. That improved visibility is essential when making detailed, balanced changes and building lasting support. This is essential for structural cartilage grafting to correct airway issues and tip refinements.
Why I Perform Only Open Rhinoplasty
Key advantages of the open technique include:
- Superior visibility for detailed reshaping: The open approach gives me an unobstructed view of the nasal framework, which is critical for making precise adjustments to both the bridge and tip of the nose.
- Greater surgical precision: Whether performing a first-time rhinoplasty or revising a previous surgery, having full access allows me to shape and support the cartilage with more control and accuracy.
The open method facilitates precise graft placement and enhances structural support, helping your results maintain their shape and integrity over time.
But What About the Scar?
One of the most common concerns I hear about open rhinoplasty is the external incision, and I understand why it gives some people pause. But in reality, it’s a very minor issue for most patients.
- The hidden gullwing incision is located on the columella, a naturally shaded area at the base of the nose.
- It typically heals into a fine, barely visible line.
In my experience, patients are pleasantly surprised by how well the incision fades—and how worth it the trade-off is once they see their final results.
Is Open Rhinoplasty Right for You?
