What Is Rhinoplasty Surgery?

Rhinoplasty is a surgical procedure that addresses cosmetic and sometimes functional concerns about the nose. More than 300,000 men and women choose rhinoplasty each year to create a more proportional, shapely, or functional nose.

The skin is then re-draped over the new framework, and the incisions are closed with sutures.

Proven Techniques, Outstanding Results

Dr. Gross’ background in reconstructive facial surgery and additional board certification in otolaryngology (ENT) make him uniquely qualified to perform rhinoplasty and reconstructive rhinoplasty procedures. He achieves a number of goals with rhinoplasty, including:

  • Correcting an excessively long, short, crooked, or wide nose
  • Restoring breathing function (septoplasty)
  • Correcting a deviated septum or turbinate cautery
  • Reducing the appearance of a dorsal hump
  • Elevating, lifting, shrinking, or shortening the nasal tip
  • Improving the shape of the nose with cartilage grafts or nasal implants
  • Narrowing the nostrils

Dr. Gross routinely performs osteotomies to reset the nasal bones and bring in the sidewalls to narrow  the nasal bridge. He performs tip plasty to refine, lift, and shorten the tip of the nose and is an expert in this area. Dr. Gross also recommends patients use a product called Nose Perfect® during their recovery to help minimize swelling in the tip or if they have thick nasal skin.

For those who have had facial trauma, Mohs surgery, or congenital nose issues, Dr. Gross meticulously performs reconstructive rhinoplasty to reform the function or appearance of the nose. He frequently receives referrals from other plastic surgeons, ENT surgeons, and oral surgeons for his experience handling complex cases.

Ideal Rhinoplasty Candidates

The best candidates for this procedure have a BMI below 30, enjoy good overall health, and refrain from smoking. While there are no set age requirements for this procedure, patients should wait until their noses are finished growing before pursuing rhinoplasty. For girls, this occurs at about age 15 or 16; for boys, the nose stops growing at age 17 or 18.

Combining Procedures

Typically, rhinoplasty is a standalone procedure, but some people may opt to combine it with other facial procedures such as eyelid surgery, chin augmentation, or facelift surgery.

In this type of procedure, Dr. Gross minimizes the amount of hard and soft tissue removed to maintain the structural integrity of the nose and avoid undesired side effects of a collapsed bridge, nasal valve, or over-rotated tip. Tip Plasty

Our Computer Imaging System vs. a Rhinoplasty (Nose Job) Filter or App

Our practice is among the few that use advanced computer imaging to show potential rhinoplasty results. With the help of this technology, Dr. Gross can reliably tailor a surgical treatment plan for each patient’s rhinoplasty needs. Phone apps and filters can be fun but suffer in accuracy because the input has the following limitations (learn more):

  • Single camera angle
  • Fixed lighting source

With more than 2 decades of experience using computer imaging, Dr. Gross produces results that closely match the previews provided by the imaging system. Barring any unforeseen problems, most patients will be very satisfied with the final results Dr. Gross simulates on the computer imaging system. He examines each patient individually and advises each patient about whether their expectations are realistic based on this comprehensive analysis. What Is Rhinoplasty Recovery Like?

Rhinoplasty is a 2.5-hour outpatient surgery, and you’ll be able to return home the same day. It’s important to carefully follow your rhinoplasty after care instructions for a smooth recovery.

Each patient’s recovery is unique; however, the following timeline offers a general guide of what to expect.

Day 1 to 3

Ask a trusted loved one to drive you home and stay with you for at least the remainder of the day following surgery. Since we don’t use nasal packing, most patients are surprised at how comfortable recovery is. The following tips can make recovery easier and quicker:

  • Use ice compresses on your forehead to ease swelling and discomfort.

Day 4 to 7

You’ll return to our office for a post-op cast removal appointment so we can remove your cast, nasal taping, and sutures. You can return to school or work the day your nasal cast is removed, looking quite normal and natural.

Week 2 to Year 1

You should expect your results to continue improving over the first year as rhinoplasty swelling gradually subsides week by week. Some lingering temporary numbness at the tip of the nose is not unusual.

How To Massage the Nose After Rhinoplasty

About 2 to 3 weeks after surgery, Dr. Gross may recommend compression exercises and fingertip massage of the nose to reduce swelling in certain areas. We will show you how to safely perform these exercises 2 to 3 times a day for 2 to 3 minutes, as outlined in our rhinoplasty post-op instructions.

  • What’s the best age to get a nose job?
    People can benefit from rhinoplasty surgery at nearly any age; however, you should wait until your nose is fully developed before pursuing surgery. For most people, this occurs in the teen years. For more information and before-and-after photos, read this blog post.
  • Will Dr. Gross need to break my nose during surgery?
    It depends. In some cases, a controlled break  (osteotomy) is necessary to adequately reshape the nose. However, this is not always the case. For some patients, subtle bone reshaping is possible without a break. In many cases, it is necessary to correct a wide nasal bridge or to straighten a crooked nose from prior trauma. During your consultation, Dr. Gross evaluates your nose and discusses your goals with you to determine whether osteotomies are necessary. Resetting your nose will not significantly lengthen your recuperation. It is a common and safe method, particularly in Dr. Gross’ experienced hands.
  • When can I get back to the gym?
    Although you can return to work about 1 week after surgery, it’s important to give yourself a bit more time off from vigorous activities, including exercise. This is because activities that quicken your pulse also raise your blood pressure, potentially leading to uncomfortable swelling and irregular scarring. In most cases, Dr. Gross recommends holding off on exercise for at least 2 to 4 weeks, gradually increasing your activity level as you heal.
  • Is rhinoplasty surgery painful?
    Many patients are pleasantly surprised to learn that recuperation following rhinoplasty is not particularly painful. Although discomfort is normal following any type of surgery, including rhinoplasty, it is generally manageable with prescribed medications. Many patients experience headaches and a feeling of sinus pressure or nasal congestion. These are normal and resolve as swelling gradually dissipates.
  • When can I wear glasses again after rhinoplasty?
    Typically, rhinoplasty patients must avoid wearing glasses for several weeks following the surgery. An innovative product called RhinoShield® is now available at our practice that enables patients to resume wearing glasses almost immediately. The product protects the soft cartilage from glasses so they don’t create permanent indentations to the bridge of the nose.
  • Where are the incisions placed during nose surgery?
    Dr. Gross hides most rhinoplasty incisions inside the nostril. External incisions may be hidden in the creases where the nose meets the cheek. He also makes a thin incision under the nasal tip (on the columella) for open approach rhinoplasty. This incision heals beautifully, leaving no visible scar.  Sutures inside and outside of the nose dissolve after the procedure.
  • What type of cast will I have over my nose?
    A nasal cast (Denver splint) made of aluminum and foam is held in place with Steri-Strips. You must keep the cast and tape completely dry for the entire week. The nasal cast applies compression to the nose and protects it during the week of healing.
  • Are there nonsurgical alternatives to rhinoplasty surgery?
    For some people, a nonsurgical nose job with dermal fillers is a good way to temporarily disguise minor nose concerns such as a small hump or downturned tip. Often called a liquid rhinoplasty, this procedure typically lasts about 6 months and—for safety reasons—requires an injector with extensive knowledge of nose anatomy.
  • Does thread rhinoplasty work?
    Thread rhinoplasty, also called a nose thread lift or string rhinoplasty, is another nonsurgical alternative to rhinoplasty. This procedure is said to raise the tip of the nose by tying a string around the end of the nose and around the back of the head. This procedure provides no long-term improvement and could lead to serious complications. Learn more in this blog post.
